HertsOne was created in 2017 as a not-for-profit GP Federation by the 53 practices covering South and West Hertfordshire, supporting a population of over 600,000 people.
What do we do?
We were established to fill a gap in service provision, using GPs, nurses and pharmacists with specialist skills, we provide Primary Care at-scale that support patients whose needs are outside of the scope of General Practice, helping manage complex need that reduces the pressure on our already busy hospital and specialist services.
We are fully embedded in our local health and care system and can mobilise quickly and efficiently to deliver innovativemodels of care.
We are keen to do more to help reduce the pressure on the NHS and have a tried and tested operating model that is both efficient and effective at providing care.
